The basic requirements for a massage therapist license are as follows:

1. Completion of not less than 500 hours in specific course work required in Code Section 2-3A-4. Proof must be by certified transcript.

2. Successful completion for certification in first aid and in cardiopulmonary resuscitation by the American Heart Association or equivalent.

There are continuing legal education requirements:
All licensed massage therapists applying for a renewal license must provide proof that they have received a minimum of twelve hours (50 minutes per hour) of continuing education during the preceding 12 months. One hour of continuing education credit will be awarded for each hour training relating to those items specifically listed in Code Section 2-3A-6 and C.P.R.

When making
application the following is required:
1. Name, home address, home phone, description, including height, weight, race, and color of hair and eyes.
2. Written proof that the applicant is at least 18 years of age.
3. Business, occupation, or employment of the applicant for the 3 years preceding the date of application.
4. Any criminal convictions, other than traffic infractions, including the jurisdiction in which the offense took place; the sentence or other penalty, if any; and the current status of the case.
5. The position or function the applicant is expected to perform within the business.
6. Proof of completion of the educational requirements for license.
7. Authorization for City, its agents and employees to conduct an investigation into the truth of the statements set forth in the application and the qualifications of the applicant.

FEES:
The nonrefundable fee for licensing a massage therapy establishment is paid per year and is payable upon application. The fee for licensing the establishment does not include licensing for individual employees. Each employee must be licensed separately, with a nonrefundable fee due at the time of application.
